Registration for Research Day - Tuesday 6th March
Payment: This conference is free of charge, but to ensure a full delegate attendance we ask that you please make a cheque payable to SDCRN for £25 and return it with this completed form. The cheque will be returned to you on arrival at the conference.
Cancellation Policy: Should you require to cancel your booking please notify Philip Brown on philipbrown1@nhs.net or 01738 562322. If you cancel before 24th February 2012 your cheque will be returned to you. If cancellation occurs after 24th February 2012, the cheque will be forfeited. A substitute delegate can be named at any time free of charge.
